Program Requirements: Project MEND's services
assist low-income individuals with disabilities obtain medical
equipment and specialized fitted devices. Since eligibility
for our services is determined based on income, applicants
will need the following information:

Other official city, state, or federal documents
2. Current Texas Address
Current utility bill
Texas Driver's License with current address
Bank or credit card statement
IRS documents
Other official documents
3. Income (applicant only; if applicant is 0-18 years
of age, then income for parent(s) living in the household
is required)
Most recent check stubs
Bank statements
Income tax return
Social security payments
Supplemental Security Income (SSI)
Social Security Disability Insurance (SSDI)
Temporary Assistance for Needy Families (TANF)
Aid to Families with Dependent Children (AFDC)
Pension benefits/payments
Retirement benefits/payments
Other sources of income
4. Picture Identification
Driver's License
State Issued ID Card
VISA/Passport
Military ID
5. Doctor's prescription for medical equipment or specialized
fitted device being requested
Prescription issued by applicant's Physician, Licensed
Medical Professional, or Hospital
