Eligibility for Project MEND Services

Project MEND's services assist low-income individuals with disabilities obtain medical equipment and assistive devices specialized and/or fitted to the individual. Eligibility for Project MEND services is based on the HUD Income Poverty Guidelines. Applicants must provide the following documentation before service can be provided.


You will need proof of the following: Documents you can use to prove eligibility:


1. Legal Residence (if applicable; if not a legal resident, all other documentation must be provided before an appoitment can scheduled)

  • Social Security Card
  • IRS documents
  • Hospital Discharge Form
  • Voter Registration Card
  • Other official city, state, or federal documents


2. Current Texas Address

  • Current utility bill
  • Texas Driver's License with current address
  • Bank or credit card statement
  • IRS documents
  • Other official documents


3. Applicant Income (Proof of income is needed for the applicant only. Household income is NON needed. If the applicant is 0-18 years of age, then income for parent(s) living in the household is required)

  • Most recent check stubs
  • Bank statements
  • Income tax return
  • Social security payments
  • Supplemental Security Income (SSI)
  • Social Security Disability Insurance (SSDI)
  • Temporary Assistance for Needy Families (TANF)
  • Aid to Families with Dependent Children (AFDC)
  • Pension benefits/payments
  • Retirement benefits/payments
  • Other sources of income


4. Age/Birthdate

  • Driver's License
  • State Issued ID Card
  • VISA/Passport
  • Military ID
  • Birth Certificate
  • Baptismal Certificate


5. Need for the medical equipment or assistive device being requested


  • Prescription issued by applicant's Physician, Licensed Medical Professional, or Hospital



6. Military Status (Required for veterans, active military personnel, their dependents and spouces)

*** Eligibility for military personnel, veterans, their family members and spouces is not based on income. Only proof of military status, picture ID, and doctor's prescription for the medical equipment or assistive device is needed.
